Outcomes Assessment Tools
A logic model is a table that explicitly relates the inputs, activities, outputs, and outcomes of a project in a systematic way. The template below includes notes for each column header that not only explains the purpose of each column but also presents examples. Additionally, the template includes a complete example with all columns filled out; note that the example has short-term, medium-term, and long-term goals but some models omit one or more of those columns depending on the length of the project.
An evaluation matrix is similar to a logic model in that it is a table that systematically maps out important aspects of a project and documents how they are related. It differs in its strong focus on program evaluation, the systematic process by which you can understand and document the success of a program as determined by the available data and the specific goals of the program. The template below includes notes for each column header that not only explains the purpose of each column but also presents examples.
